Confidentiality Agreement:

Patients receiving health care services have a right to expect that the confidentiality of individually identifiable medical or personal information in any form or medium will be reasonably preserved and secure. 
It is the obligation of each employee, volunteer, student, consultant, extended caregiver, member of the Medical Staff and Administration to protect the confidentiality of any patient private information regardless of it's source.  
This trust should not be broken by disclosing this confidential information to individuals that do not have a clinical or business reason to possess the information. You are responsible for safeguarding this information whether it is in electronic and/or paper report format.
